What you'll accomplish

By the end of this guide, you'll be able to speak your clinical observations at the bedside or immediately after care — and get back formatted nursing documentation. Instead of reconstructing what happened from memory at the end of a 12-hour shift, you capture it in real time while the details are fresh.

What you'll need

  • A smartphone (iPhone or Android)
  • Internet connection (facility WiFi or cellular)
  • The Nuance DAX Express app (free download)
  • Time needed: 20 minutes to set up, 2 minutes per documentation session
  • Cost: Free (DAX Express); professional plans available for facilities

How-To Guide: Use Nuance DAX for Voice-First Clinical Documentation

Step 1: Download and set up DAX Express

  1. On your phone, open the App Store (iPhone) or Google Play (Android)
  2. Search for "Nuance DAX Express"
  3. Download and install the app (it's free)
  4. Open the app and create an account using your email
  5. During setup, select your role — choose Nurse or the closest option available

What you should see: A simple recording interface — a large microphone button in the center of the screen.

Troubleshooting: If you can't find DAX Express, search for "Dragon Medical One" or "Nuance Medical" — the naming has evolved. The free tier app is specifically "DAX Express."
